Manager, Aquisitions
We have an exciting opportunity for a Manager, Care Partnering Acquisitions. This role is responsible for leading a team who monitor for potential and new clients moving into the Support at Home program. Working within the rights-based, person-centred framework, the Manager Care Partnering, Acquisitions facilitates new client introductions into Silverchain as a comprehensive Aged Care community provider along with transitioning existing clients into higher levels of funding. In partnership with the broader WA Aged Community Care team, the role plans, motivates and leads the growth of Support at Home clients.
The Role
In addition to leading a high-performing team to onboard clients—driving conversion, and consistently delivering against organisational KPIs, this role will also:
- Lead and support a team of Customer Service Administrators and Client Service Coordinators, providing guidance and oversight of quality service delivery which aligns with best care and aged care quality standards.
- Ensure adequate frameworks are in place to support the identification of potential and current clients pending or assigned Support at Home funding.
- Have a strong understanding of Support at Home funding and guidelines, to develop and review work instructions which align with legislation and funding guidelines.
- Develop plans and strategies to motivate team achievement of key performance indicators, expanding our customer base and ensuring a strong presence.
- Track key metrics to understand onboarding and separation rates and customer experience.
- Understand and build positive customer relationships to support their choice when choosing an Aged Care providers and understand how to navigate the aged care system, funding and services.
- Encourage the completion of environmental assessments in accordance with occupational, health and safety guidelines, identifying and addressing hazards and/or coordinating additional equipment, to ensure a safe environment for future care providers and clients.
- Liaise with clients, carers, families, and internal stakeholders to understand and ensure the provision of person-centered care.
- Ensure the maintenance of effective records aligned with Silverchains policies and procedures, relating to client onboarding and setup.
- Collaborate with appropriate leaders across Silverchain, to ensure effective growth of the Support at Home client demographic.
- Maintain sufficient knowledge, training competencies, and physical capacity to effectively respond to client emergencies, incidents, workforce shortages and/or to enable the provision education to direct-care providers.

What we’re looking for
The successful applicant will have the ability to translate their passion for enabling clients to live their best life as they choose into business practices and models that optimise client satisfaction and, in addition to this, will possess the following skills, experience and attributes:
- Demonstrated knowledge, experience and understanding of aged community care to effectively lead and coordinate suitable on-boarding activities
- Demonstrated knowledge and experience of leading a team
- Demonstrated understanding of the Support at Home Program, Quality Standards, and aged care rights
- Experience working in complex, fast paced environments with high priority and time-sensitive service delivery requirements
- Well-developed communication (written, verbal and listening) and interpersonal skills to develop effective relationships, promote services and resolve client/staff issues.
- Demonstrated ability to build and maintain effective working relationships with internal and external stakeholders with varying communication skills and understanding of Support at Home program.
- A demonstrated empathetic and flexible approach to manage the sensitivity of client and employee issues.
- A flexible approach together with decision-making, negotiation, influencing and advanced problemsolving skills to innovatively manage care coordination
- Demonstrated computer literacy in word processing, spreadsheets, and calendar management software to prepare correspondence and effective documentation.
- Demonstrated knowledge of manual handling principles with the ability to train others manage service operations within a competitive and growing industry
- Tertiary qualifications in a relevant discipline is desirable
