The Opportunity

 

Silverchain are recruiting Registered Nurses to support a new chronic disease service across the Blacktown and Cumberland LGAs with Wentwest. The role will focus on assessment, care planning and coordination for people living with complex chronic conditions, working closely with GPs, practice teams, and specialist services.

 

 

About You

 

  • Relevant qualification in Nursing with demonstrated experience in field of expertise
  • Experience in delivering chronic condition management, health care navigation, health coaching and the principles of health literacy
  • Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families, and other health care professionals
  • Demonstrated ability to provide education, support and coaching to clients and their carers
  • Demonstrated time management and organisational skills to effectively plan and prioritise work
  • Demonstrated ability to work both autonomously and with a team to participate and contribute to program and organisational objectives
  • A demonstrated empathetic and flexible approach in all interactions with clients, carers and other health providers
  • Ability to work autonomously or with minimal direct supervision
  • Based on Silverchain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19 including booster, or evidence of a medical exemption in order to commence employment with Silver Chain.
