Silverchain are seeking a Level 2 Liaison Nurse to champion our services, drive referrals and deliver seamless client care across the continuum.

 

In this pivotal role, you’ll connect hospitals, clients, families and community providers, acting as the key link between allocated Perth Metro hospitals and Silverchain WA, representing our services with confidence and credibility. 

 

This is a full-time role (Monday–Friday, 8:00am–4:00pm), based across Perth Metro hospitals with occasional meetings in Osborne Park.

 

We’re looking for an experienced Registered Nurse with exceptional communication skills and a natural ability to influence and build relationships. You’ll confidently engage a wide range of stakeholders, provide clear education and information, and play a critical role in enhancing client pathways and care experiences.

What we’re looking for

 

  • Minimum 5 years’ Post Grad nursing experience
  • Demonstrated advanced knowledge of community-based services, care requirements and service capabilities.
  • A degree of knowledge regarding hospital and community care processes, systems and facilitate accurate communication between all parties.
  • Experience of working with clients with complex medical and Social behavioral conditions.
  • Current Registration with the AHPRA
  • The ability to work effectively within a multi-disciplinary team(s)
  • Compassion and enthusiasm, with a friendly approach
  • A current full driver’s licence
