Care Co-ordinator - Registered Nurse

This role is part of a team that will provide Care Co-ordination, across South Eastern NSW. Ideally you will be living in Wollongong and will cover the Illawarra area.

What we’re looking for

  • Bachelor of Nursing with current AHPRA registration.
  • Experience in delivering chronic condition management, health care navigation, and coaching.
  • Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, and other health care professionals
  • Demonstrated time management and organisational skills to effectively plan and prioritise work
  • Demonstrated ability to work both autonomously and independently
  • Demonstrated ability to function within a team, collaborating with colleagues and participating in service development.  
  • Based on Silverchain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19 including booster, or evidence of a medical exemption in order to commence employment with Silverchain.

 

The role

 

We are seeking experienced Registered Nurses who will be responsible for care coordination, navigation, and heath coaching services via face to face or telehealth to clients with chronic conditions.

This role will involve developing individualised care plans, referring to other providers as needed, navigation across multi-disciplinary services and supporting self-management principals.
