The opportunity
Working within the Aged Community Care portfolio, Silver Chain is currently recruiting for Assistant Care Team Leader to join their East team. Fleet car will be available for this role.
Assistant Care Team Leaders will be working in the community covering Ascot, Belmont, Kewdale, Bentley, Como, South Perth, Waterford, Willetton and Wilson
Monday to Friday 7am-3pm (as required to cover unattended services as part of the role).
This role will be providing care to clients with varying needs and support to the CTL's. Suitable candidates should be experienced with all levels of care including low, moderate, and complex care, and be comfortable to undertake duties with a variety of clients.

- We are looking for those with a Certificate 3 in Individual Support or equivalent (or, be working towards this qualification)
- Experience working in the community
- Hold a full Australian driver’s licence and own transport for work purposes
- You will be responsible for mentoring other direct care employees and provide support to other internal stakeholders as required
- You will have sound knowledge of best practices whilst working in the community, possess strong interpersonal skills and be flexible in your approach
Key Responsibilities
- Deliver care and support to clients in accordance with the care plan and Silver Chain Groups policies and procedures; communicate any changes to client’s condition that require review of care.
- Develop and maintain an awareness of the emotional and physical needs of the individual to ensure appropriate person-centred services is provided.
- Be responsive to the specific needs of clients, e.g. dementia and disabilities; to ensure that appropriate care and support is delivered.
- Support training and development of new employees as required by interpreting and explaining relevant process’ and requirements of the business and individual roles.
- Assist the organisation by taking responsibility for the identification, reporting, management and/or elimination of hazards and unsafe work practices as required for after-hours incidents, providing operational guidance and suggested improvements including escalation to Care Team Leader when required
- Develop and maintain inclusive and effective communication, assisting with employee queries and/or complaints to ensure consistent and effective outcomes for the service delivery, incident management and client care, escalating any concerns to the Care Team Leader or Community Care Manager as required.
- Monitor client services to ensure they are delivered effectively and are achieving the objectives of the care/support plan in collaboration with Care Team Leader and/or Home Care Package Coordinator.
- Undertake low risk, CHSP client reviews, updating care/support plan as required, escalating any areas of concern to the Care Team Leader and/or Community Care Managers as required.
