Transition Care Program Lead
Silverchain are recruiting for this recently created and exciting role in our organisation.
Suiting an experienced Registered Nurse, OT or Physio, who is keen to develop/grow their leadership abilities, while continuing to use their clinical knowledge and skills, our Transition Care Program Leader, is responsible for the coordination and delivery of care for our TCP service.
Although the preference is full time, (Monday to Friday, 8am - 4pm) we can talk about flexibility for the right applicant. Some of the role responsibilities will be to:
The Role
Maintain a level of operational TCP knowledge and relevant Health Department guideline updates.
• Lead, delegate and organise effectively the delivery of the Transition Care Program Service including internal and external stakeholder engagement, assessment, care and discharge planning, and reporting requirements.
• Initiate and lead continuous improvement requirements relating to the TCP program and broader multidisciplinary care team.
• Promote and support a positive culture with demonstrated professional behaviours in line with Silverchain’s Code of Conduct and Best Care Framework.
• Coordinate and deliver client care in accordance with current professional and ethical standards, evidence-based practice, relevant legislation, and common law.
• Formulate and maintain client support plans in consultation with the client and their family, which address the holistic needs of the client throughout the service delivery period.
• Act as an advocate for the client and their family whilst protecting and upholding their rights in relation to the care assessed and provided by Silver Chain.
• Be accountable and responsible for all care given including; assessments, maintenance of support plans; managing emergencies; and identification and delivery of appropriate interventions for complex client problems.
• Act as an escalation point for client care within the TCP service delivery.
• Collaborate with and seek advice and assistance from the Multidisciplinary Care Leadership team.
• Assess client and carer needs for education / support, inform them of additional resources as appropriate and monitor the effectiveness of the intervention; adjust interventions where relevant.
• Actively participate in education and professional development of self and others.
• Accept feedback and participate in preceptor programs at appropriate level and where required, act as an effective preceptor.
• Ensure contract compliance with managing TCP Health Department portals, including monthly data reporting (if required) against our reportable KPIs.
• Current nursing registration with Australian Health Practitioner Regulation Agency with demonstrated high level clinical skills and minimum 2 years post grad experience.
• Demonstrated knowledge and ability to use appropriate treatment and assessment methods.
• Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families and direct care employees.
• Demonstrated ability to provide education, support and supervision to direct care employees, clients, and their carers.
• Demonstrated sound decision making and problem-solving skills to innovatively manage the provision of quality client care.
• Empathetic and flexible approach to manage the sensitivities of client and employee issues within a clinical environment.
What we are looking for
• Current nursing OR Allied Health registration with Australian Health Practitioner Regulation Agency with demonstrated high level clinical skills and minimum 2 years post grad experience.
• Demonstrated knowledge and ability to use appropriate treatment and assessment methods.
• Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families and direct care employees.
• Demonstrated ability to provide education, support and supervision to direct care employees, clients, and their carers.
• Demonstrated sound decision making and problem-solving skills to innovatively manage the provision of quality client care.
