Silverchain is seeking experienced Home Care Workers to join our Transitional Aged Care Program (TACP). The TACP program supports older people transitioning from hospital back to independent living at home. This short-term, rehabilitation-focused program assists clients recovering from extended hospital stays and helps them achieve their recovery goals. We are looking for TACP Home care workers to join Silverchain team in the Hornsby area and Surrounds.

What’s on offer

  • Part time Award rate $37.53 + Super + Km’s

  • Paid CPR and First Aid Courses.
  • Monday to Friday roster - between hours of 7 am and 5pm (please note: must be available 2-3 days a week).

What We’re Looking for

  • Certificate III or above in Aged Care, Individual Support
  • Level 3 Home Care Worker experience preferred
  • Minimum 6 months' experience in a similar role
  • Current Australian Driver's Licence
  • Reliable vehicle with comprehensive insurance
  • Ability to meet all mandatory vaccination requirements (e.g  Diphtheria, Tetanus & Pertussis (Whooping Cough); Hepatitis B; Measels, Mumps & Rubella (NMR); Varicella (chickenpox); Tuberculosis (TB)**; Influenza and Covid -19)
  • Experience supporting high-care or post-hospital clients highly regarded

The Role

In the role of TACP Home worker you will be providing our aged care clients with home help services post hospitalisation, such as:

  • Personal Care (PC)
  • Exercise program support, as directed by Allied Health
  • Assistance with compression stockings
  • Transporting clients to appointments
  • Social support and engagement
  • Meal preparation
  • Domestic Assistance (DA)
