Community Care Manager

 

We are looking for a dynamic Community Care Manager to lead, support and manage our CHSP teams, including material and financial resources to achieve business objectives that drive best practice and ensure quality outcomes for staff, clients and carers within the community.

This is a full-time role, Monday to Friday. We offer working hours of 7am – 3pm or 8am -4pm (Flexible) Office base is Osborne Park (with flexible working arrangements offered)

The Role

 

  • Regular communication with the Regional Manager.
  • Drive results and growth for the Service Centres.
  • Lead and manage quality within service delivery to ensure Silver Chain meet client needs and goals while looking for effective ways to improve service delivery.
  • Build relationships and network with internal & external parties to promote services and identify opportunities.
  • Decision making and problem solving.
  • Monitor client case management process to ensure all activities are carried out in a timely manner and meets changing client requirements.
  • Measure, support, coach and develop employee performance.
  • Monitor employee workload and feedback to Business Services team to assist with appropriate resource utilisation and forecasting recruitment needs.
  • Maintain inclusive and effective regular communication with your team to ensure employees are aware of information such as business objectives, organisational strategies and expectations.
  • Monitor, evaluate and review Quality Management Systems to ensure standards are met and maintained, and processes are efficient and cost effective.
  • Manage and monitor compliance to policies, procedures, contracts, accreditation and legislative obligations to ensure quality of services, work practices and alignment across Service Centres.
  • Monitor, evaluate and action agreed KPIs to meet organisation and business unit objectives.

The successful applicant will have the ability to translate their passion for enabling clients to live their best life into business practices and models that optimise client satisfaction.

In addition to this, will possess the following skills, experience and attributes:

  • Demonstrated decision making, negotiation and advanced problem-solving skills
  • Strong knowledge of funding streams, in particular Support at Home and CHSP.
  • You will work autonomously and be confident escalating issues you may require support from Management.
  • Be able to drive results in order to deliver KPIs
  • Demonstrated leadership abilities to lead, mentor, support, manage and coach
  • A commitment to excellence in customer service to internal and external stakeholders.
  • Demonstrated experience managing physical and financial resources
