The opportunity

As a Case Co-ordinator, you will have a unique opportunity to provide clinical leadership to a team of community nurses to deliver high-quality, client-focused services, Monday to Friday between the hours of 7am and 4 pm.

You will lead your team to meet the needs of clinically complex acute and aged care clients by providing integrated community care.  You will contribute to the daily operations of clinical service through coordination of clinical and physical resources and by provision of direct client care.  You will provide a key escalation point for nurses requiring advice on management of client deterioration and other clinical challenges.  You will support the Clinical Nurse Manager to meet service delivery goals and contractual obligations, following RDNS policies and procedures.

About you

To be successful in this role, you will meet the following criteria:

  • Bachelor of Nursing with current AHPRA registration.
  • 5 years post graduate nursing experience.
  • Demonstrated experience in clinical leadership and coordinating resources to achieve outcomes.
  • Demonstrated ability to meet KPIs.
  • Experience in mentoring both new and existing staff in their clinical roles and professional development.
  • Demonstrated decision making skills to innovatively manage the provision of quality client care.
  • Well-developed communication and interpersonal skills, with capacity to negotiate, problem solve and influence individuals and organisations.
  • Demonstrated ability to work autonomously with a team focus.
  • Applicants must be comfortable navigating change and rapidly acquiring new skills.
  • A current Australian driver’s licence.
