The opportunity

Are you a skilled Registered Nurse, who is passionate about palliative care and ready to take the next step in your career? We are seeking a Case Coordinator, Palliative Care to ensure our dedicated team provide a quality service and make a difference to the lives of our palliative care clients and their families.

 

This role operates across Monday to Friday, between 7am and 4pm.

Key Responsibilities

  • Lead and support a dedicated team of Registered and Enrolled Nurses in a community setting, fostering growth, competence, and emotional wellbeing
  • Coordinate resources to ensure quality person-centred care
  • Oversee clinical and other care needs for community Palliative Care clients, including complex clinical issues
  • Work closely with other teams to ensure effective delivery of palliative care services
  • Act as a key escalation point for nurses managing client deterioration and other clinical challenges
  • Drive communication through team meetings, stakeholder engagement, and documentation excellence
  • Support onboarding, mentoring, and student placements to build a strong, future-ready workforce

About you

You have passion with purpose and are driven by excellence. Caring is a profession and a calling, and your work and support enriches everyone. In addition, you will have the following skills:

  • Current AHPRA registration, with a minimum 2 years experience in Palliative Care
  • Demonstrated clinical leadership and resource coordination
  • The ability to make innovative decisions to support quality client care
  • Skilled in mentoring staff and supporting professional development
  • Excellent communication, negotiation, and problem-solving skills
  • Comfortable working autonomously and collaboratively in dynamic, changing environments
  • A current Australian driver’s licence
