Our Community Specialist Palliative Care Service supports clients and their families with life limiting illnesses at home.

 

We are seeking an experienced Social worker to join our team on a Permanent Part Time basis 0.8 FTE (4 days per week) the days of the week are flexible (within Monday-Friday) and hours are 8.30am – 5.00pm. You will be seeing clients around Perth Southern Metro Suburbs including Peel and there will be an expectation to cover leave in the northern suburbs.

 

You will work closely with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with an ultimate goal of reducing the number of unplanned admissions to hospitals by successfully integrating the care needs of each client.

What we’re looking for

  • Tertiary qualification in Social Work and eligible membership of AASW (Australian Association of Social Workers).
  • Previous experience working within palliative care and/or health service.
  • Current Working with Children’s Check.
  • The ability to work within an interdisciplinary team.
  • Experience in client assessments and care plans.
  • High computer literacy.
  • Current Working with Childrens Check.
  • Great organisational and time management skills.
  • Compassion and enthusiasm, with a friendly approach.
  • Excellent Communicational skills both Verbal and written.
  • A current Full Australian driver’s licence.
