Are you a skilled Registered Nurse ready to take the next step in your career? Silverchain is seeking a passionate Case Coordinator to lead a clinical team in delivering integrated community care to clients with complex acute and aged care needs. In this pivotal role, you will coordinate clinical and physical resources, provide direct client care, and serve as a key escalation point for clinical challenges. You will also collaborate closely with the Clinical Nurse Manager to achieve service delivery goals and uphold Silverchain's commitment to excellence.
This role operates across Monday to Friday, between 7am and 4pm.
